LIST: Online registration forms of Metro Manila cities for COVID-19 vaccination

MANILA, Philippines — To ensure a smooth rollout of COVID-19 vaccines, several local government units (LGUs) in Metro Manila have launched their online registration platforms.

Most online registration forms require the sector from which the registrant belongs to, their address, identification card numbers, medical histories, and previous diagnosis for COVID-19, if any.

Las Piñas City

Visit this to access the Google form from Las Piñas City for the vaccination registration.

Upon clicking the link, you will be asked to identify the sector you belong to, contact number, address, medical history, COVID-19 infection history, and other details.

Registrants will likewise be asked for a 1×1 picture in white background and a photo of your government-issued ID. Click “submit” after filling out all the required fields.

Muntinlupa City

Visit to access the COVID-19 vaccine pre-registration form of Muntinlupa City. In the webpage, you will need to input your personal information, allergies, and medical history. 

Navotas City

Parañaque City

Visit to register. Sign up by providing your email and password or by linking your Facebook account. You will then be asked to provide your mobile number and to upload your photo.

Upon signing up, click “register now” on the next webpage that will appear, input the necessary details, then click “submit.

San Juan City

Visit to register for San Juan City’s vaccination drive. 

 Upload a photo of your valid ID and 1×1 picture, and fill out the required details. Click “submit” upon completing the form.

Valenzuela City

Your log-in details that will be used in registration are the same as those you used for the ValTrace QR Code.

Fill out the required fields then submit the completed form.

Mandaluyong City

The registration form for Mandaluyong City’s vaccination can be accessed through this .

The website will require your personal details and your photo. Click “register” upon completing the form.

Pasig City

In February, Pasig City Mayor Vico Sotto said the LGU would use its “PasigPass” app, the city’s QR Code contact tracing solution, for the COVID-19 vaccination program instead of asking residents to register again for the inoculation drive.

Taguig City

Also in February, Taguig City Mayor Lino Cayetano said the LGU’s Taguig Registry for Assessment and City Engagements, which can be accessed through , has expended from being a contact tracing tool into a registry for other services such as vaccination, other health benefits, and social services.

Quezon City

There are two ways to register for COVID-19 vaccination in Quezon City, according to a Facebook post of the city government.

The first is through online self-booking, wherein you will be required to sign up as a patient at the .

The EZConsult App can also be downloaded in the App Store or Google Play.

According to an advisory from the QC government, “your account will be under review until you are eligible based on the DOH priority list.”

QCitizen ID will not be required in registering through EZConsult, but your email address and mobile number will be needed for registration.

The other way is through assisted booking, which will require your QCitizen ID.

Under this procedure, your barangay will schedule your COVID-19 vaccination based on the priority group you belong to. A representative of the barangay will contact you and provide your vaccination schedule.

QCitizen ID registration sites at the Quezon City Hall, Barangay Halls, and QCitizen ID kiosks are currently closed due to the ongoing enhanced community quarantine, but online registration can be done through .

Caloocan City

The pre-registration form of Caloocan City can be accessed through this . However, as of March 29, the website says that the form is no longer accepting responses.
